Choose to Change
Referral Criteria
Please note that we only accept professional referrals for our Choose to Change programme. Please see our inclusion & exclusion criteria below to see if your child is eligible for this programme.
Inclusion Referral Criteria
- Children aged 1 – 17 (up to 18th Birthday) with a BMI above the 85th centile.
- Referral will also be accepted for children whose BMI is less than 85th centile if you have a concern about the child’s weight (please provide details of concern).
- Children must be a resident of Nottingham City or Nottinghamshire County and/or be registered with a GP in Nottingham and Nottinghamshire.
- For children under 16 years old they must be supported by the Parent/Guardian/Carer to attend the weight management programme.
Exclusion Referral Criteria
- Children not residing in Nottingham City and Nottinghamshire County or not registered with a GP in Nottingham and Nottinghamshire.
- Children under the 85th centile (unless you have a weight related concern).
- Age (over 18 years of age)
To refer a/your child into a weight management service across Nottingham and Nottinghamshire a referral must be completed by a professional. The child will be triaged into the most appropriate service for them. If you feel a child is eligible for our service, please speak to a professional and inform them of your interest in our service to get started. Please see below for a list of eligible professionals.
